What is PHI?
Protected Health Information (PHI) is any information about your health, health care, or payment for care that can identify you. Examples include symptoms, diagnoses, medications, insurance details, and treatment records.
What this site does NOT collect
This website is a marketing and information site only. It does not collect, store, or transmit PHI. Specifically, do not use this site to send:
- Medical records or images
- Symptoms, diagnoses, or treatment details
- Insurance policy numbers or ID cards
- Social Security numbers
- Credit card or payment details
- Any other sensitive health information
What this site may collect
- Contact information you choose to share. If you call, email, or submit a contact request, we receive whatever you provide (typically your name, phone number, and a general reason for reaching out). This is not PHI unless you include health details — please do not.
- Basic technical logs. Our hosting and analytics tools may log IP addresses, browser types, and pages visited for security, performance, and debugging. These are not used to build advertising profiles.
- Essential cookies only. We use only the cookies needed to load the site and remember basic preferences. We do not run advertising, retargeting, or cross-site tracking pixels.
Appointment scheduling and patient forms
When you book an appointment or complete new-patient forms, you are redirected to our practice's scheduling platform, Modento. That platform is separate from this website and has its own privacy and security practices. The information you enter there is handled by the practice according to our Notice of Privacy Practices.
