Beam Family Dentistry is committed to protecting the privacy of your health information as required by the Health Insurance Portability and Accountability Act (HIPAA) and applicable Kentucky law. A printed copy of our current Notice of Privacy Practices is available at our office.
Your rights
You have the right to request a copy of your dental records, request corrections, ask for confidential communications, request restrictions on certain uses and disclosures, receive an accounting of disclosures, and receive a paper copy of this Notice.
